Parent/Guardian Indemnity & Consent Agreement
By registering my child/ward to participate in Royal Soccer Academy programs, I, the undersigned parent/guardian, acknowledge and agree to the following:
I understand that participation in soccer involves physical activity and carries inherent risks, including but not limited to injury, collision, or accident.
I voluntarily assume all risks associated with my child’s participation in practices, games, and Academy-related activities.
By registering my child/ward to participate in Royal Soccer Academy programs, I, the undersigned parent/guardian, acknowledge and agree to the following:
I understand that participation in soccer involves physical activity and carries inherent risks, including but not limited to injury, collision, or accident.
I voluntarily assume all risks associated with my child’s participation in practices, games, and Academy-related activities.
I release and discharge Royal Soccer Academy, its directors, coaches, staff, volunteers, and facility providers from any and all liability, claims, or demands arising from my child’s participation in Academy activities, except in cases of gross negligence or willful misconduct.
I agree to indemnify and hold harmless Royal Soccer Academy against any claims, damages, or costs arising from my child’s actions or participation.
In the event of an emergency, I authorize Royal Soccer Academy staff to secure medical care for my child/ward if I cannot be reached immediately.
I accept responsibility for any resulting medical expenses.
Drop-Off & Pick-Up Responsibility
I acknowledge that Royal Soccer Academy is only responsible for my child during scheduled program hours.
I am responsible for ensuring safe drop-off and timely pick-up of my child. The Academy is not responsible for children outside of program times.
Compliance with Rules
My child/ward will follow the Academy’s rules, code of conduct, and any facility guidelines. Failure to comply may result in suspension or removal from the program without refund.
I grant Royal Soccer Academy the right to capture, use, and publish photographs, video recordings, and other media of my child/ward participating in Academy activities.
Purpose of Use:
Media may be used for Academy-related purposes, including but not limited to promotional materials, website content, social media, newsletters, and advertising.
Ownership & Rights:
I understand that all photographs and videos remain the property of Royal Soccer Academy and I waive any claims to compensation, royalties, or further approval for their use.
Opt-Out Option:
If I do not wish my child/ward’s media to be used, I will submit a written notice to info@royalsa.ca at the time of registration.
